Feedback Forms

Feedback from our Team membership is very important and is a valuable tool for the Board of Directors to know what works well and what could use improvement. We are continually trying to improve our on-going programs, your participation and feedback would be greatly appreciated. Feedback forms can be emailed to the President or dropped in the Breakers mailbox slot located in the gift shop at the Cowichan Aquatic Center.
